Overview

Your intestinal lining acts as a selective barrier, allowing nutrients to pass into the bloodstream while keeping larger molecules, bacteria, and toxins out. When this barrier becomes more permeable than it should be, molecules that would normally be blocked can cross into the bloodstream and trigger immune and inflammatory responses. This increased permeability is sometimes referred to as leaky gut.

The lactulose-mannitol challenge test is a well-established method for measuring intestinal permeability. Mannitol is a small sugar molecule that passes easily through a healthy gut lining. Lactulose is larger and should not cross easily under normal conditions. After drinking a solution containing both sugars, you collect your urine over a timed period. The ratio of lactulose to mannitol recovered in the urine indicates how permeable your gut lining currently is.

Increased intestinal permeability has been associated with a range of conditions, including inflammatory bowel disease, coeliac disease, food sensitivities, chronic stress, and the use of certain medications such as NSAIDs. It is also a topic of growing research in relation to autoimmune conditions, though the evidence for a direct causal link is still developing.

This test provides an objective, measurable assessment of gut barrier function rather than relying on symptom-based assumptions. How to prepare

Fast for 12 hours before testing (water is fine). Avoid NSAIDs, alcohol and certain medications for 48 hours beforehand. Drink the provided test solution, then collect urine samples at the times specified in your kit instructions. Avoid strenuous exercise during the collection period.

After the test

Discuss results with your practitioner. If increased permeability is confirmed, they may recommend dietary changes, specific nutrients that support gut lining repair, and strategies to address underlying causes such as food triggers or chronic stress. Follow-up testing can track whether the barrier is improving over time.

Understanding results

Your report shows lactulose recovery, mannitol recovery and the lactulose-to-mannitol ratio. An elevated ratio suggests increased intestinal permeability, meaning the gut barrier is letting through molecules that it normally would not. Low mannitol recovery on its own may point to reduced absorption. Your practitioner can interpret these results in the context of your symptoms and health history.
