Overview

Cortisol is a steroid hormone produced by the adrenal glands. It follows a strict circadian rhythm, rising sharply in the early morning and declining through the afternoon and evening. The morning cortisol blood test captures your level at or near its daily peak.

Cortisol affects a wide range of bodily functions: blood sugar regulation, blood pressure, immune response, and how the body responds to stress. Both too much and too little cortisol can produce noticeable symptoms.

Because cortisol is so sensitive to timing, collection before 10am is not optional for this test.

Symptoms

Suited to people experiencing persistent fatigue, unexplained weight changes, difficulty coping with stress, low blood pressure, or other symptoms that may point to abnormal adrenal function.

Questions

  • Cortisol peaks in the early morning. An afternoon draw produces a naturally lower result that can't be compared to morning reference ranges.
  • No fasting is required. The most important preparation is timing: before 10am.
  • Yes. Acute stress can temporarily elevate cortisol.
  • Primary adrenal insufficiency (Addison's disease), secondary adrenal insufficiency, or adrenal suppression from long-term steroid use.
  • Cushing's syndrome, physical or emotional stress at the time of collection.
  • Yes, but tell your practitioner. Steroids can suppress your body's own cortisol production.

How to prepare

Timing is crucial for accurate results. Morning cortisol (8-9 AM) measures peak levels; afternoon cortisol (3-4 PM) assesses daily rhythm. Follow specific timing instructions from your healthcare provider. Avoid intense exercise, stress, and caffeine before testing. Fasting not required but may be recommended for morning tests.

After the test

Share your results with your GP, endocrinologist, or a practitioner familiar with adrenal health. If your cortisol is outside the expected range, further testing is usually recommended.

Your test results will be available in your private dashboard. If there are any urgent issues, we'll let you know so you can follow up with your health professional.

Understanding results

Results are reported in nanomoles per litre (nmol/L). A result below the reference range may suggest reduced adrenal output. A result significantly above the range may point to excess cortisol production. A single cortisol measurement is a starting point, not a diagnosis.

Most collection centres accept walk-ins. Some may require a booking - check details when you click on your chosen location.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
