Overview

Cortisol 4pm testing captures your stress hormone level during its normal afternoon decline. Healthy adrenal function shows high morning cortisol gradually dropping through the day, reaching this mid-afternoon checkpoint significantly lower than the morning peak.

This test helps assess whether your daily cortisol rhythm is functioning as expected. Afternoon energy crashes may reflect lower-than-expected cortisol, while evening restlessness or sleep difficulty may suggest cortisol is not declining appropriately.

Most informative when compared to an 8am morning cortisol result to map the full daily pattern.

Questions

  • 4pm captures cortisol during its normal afternoon decline. Comparing afternoon levels to morning levels shows whether your daily rhythm is functioning as expected.
  • Afternoon cortisol should typically be 50 to 70 percent lower than morning levels. The pattern of decline matters more than the absolute value.
  • Persistently elevated afternoon cortisol may suggest chronic stress, Cushing's syndrome, or disruption to your daily rhythm. Your doctor can advise on further investigation.
  • Very low afternoon cortisol alongside low morning levels may suggest adrenal insufficiency. Low afternoon with normal morning levels may be a normal variation.
  • Yes. Comparing morning and afternoon levels provides more useful information than either test alone. Some doctors order both at the same time.
  • Elevated evening cortisol can interfere with sleep onset. The natural decline should reach its lowest point around bedtime.

How to prepare

Blood must be collected at 4pm (or as close as possible). No fasting required. Avoid intense exercise, caffeine, and alcohol on the day of collection. Let the pathology collector know about any medications you are taking, especially corticosteroids.

After the test

Discuss results with your doctor. If you have not already tested morning cortisol, they may recommend an 8am collection for comparison. Abnormal afternoon levels may prompt further investigation of adrenal function or referral to an endocrinologist.

Your test results will be available in your private dashboard. If there are any urgent issues, we'll let you know so you can follow up with your health professional.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
