How to prepare
Obtain the collection container and instructions from your pathology provider before starting. Begin collection after your first morning urination, then collect all urine for exactly 24 hours, finishing with your first morning urination the next day. Keep the container refrigerated throughout. Follow the timing instructions carefully, as missed samples can affect accuracy.
After the test
Discuss results with your doctor. Elevated cortisol may prompt further investigation for Cushing's syndrome. Low results may lead to assessment for adrenal insufficiency. Your doctor may refer you to an endocrinologist for further evaluation.

Understanding results
Results show your total 24-hour cortisol excretion compared to reference ranges (typically 28 to 276 nmol per 24 hours). Elevated levels may suggest Cushing's syndrome or other conditions involving cortisol excess. Low levels may point toward adrenal insufficiency. Your doctor will interpret results alongside your symptoms and may recommend further testing.
