Overview

CEA is a protein that certain cancers produce in elevated amounts. This tumour marker is most valuable for monitoring colorectal cancer after treatment, tracking disease progression, and assessing treatment response.

CEA is not a screening test. Levels can be elevated in smokers, inflammatory conditions, and liver disease. Its value lies in serial monitoring, where trends over time indicate how cancer is behaving. Rising levels during surveillance may flag recurrence before symptoms appear.

Regular CEA monitoring helps guide treatment decisions and provides an additional data point for your oncology team to track disease changes over time.

Suited to people with a history of colorectal cancer, those undergoing active cancer treatment, or anyone whose oncologist has requested tumour marker surveillance.

Questions

  • Carcinoembryonic antigen, a protein that can be elevated in certain cancers. It is most useful for monitoring colorectal cancer but may also rise in pancreatic, lung, and breast cancers.
  • No. CEA is not specific enough for screening. It can be elevated in smokers, inflammatory conditions, and liver disease. It is used to monitor known cancer, not to screen for new cases.
  • Generally above 3 to 5 ng/mL is considered elevated, but trends over time matter more than a single value. Rising levels are more significant than stable elevation.
  • During cancer surveillance, typically every 3 to 6 months for the first 2 to 3 years after treatment, then less frequently. Your oncologist determines the schedule.
  • Rising CEA levels can precede clinical signs of recurrence by months, providing an early signal for your oncology team. However, not all recurrences cause CEA to rise.
  • Smoking, inflammatory bowel disease, pancreatitis, liver disease, and other benign conditions can elevate CEA. This is why it is used for monitoring rather than screening.

How to prepare

No special preparation required. Let your healthcare provider know if you smoke or have inflammatory conditions, as these can affect CEA levels. Avoid smoking for several hours before your blood draw if possible.

After the test

Discuss results with your oncologist. Elevated or rising CEA may prompt further investigation such as imaging studies. Stable or declining levels in cancer patients may suggest a positive treatment response. Your oncology team will advise on the appropriate course of action.

Understanding results

Results show your CEA level compared to reference ranges. Elevated levels may indicate cancer progression, recurrence, or other non-cancer conditions. A single elevated reading is less meaningful than a trend over time. Your oncologist will interpret results alongside your clinical history, imaging, and other tests.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
