Overview

Manganese is a trace mineral that the body needs in very small amounts. It acts as a cofactor for enzymes involved in bone formation, wound healing, and antioxidant defence. Dietary manganese is tightly regulated by the gut and liver, so deficiency and toxicity from food alone are both uncommon.

The primary clinical use of manganese blood testing is occupational health monitoring. Workers in welding (particularly with manganese-containing rods), mining, smelting, steelmaking, ferroalloy production, and dry battery manufacturing can be exposed to manganese through inhalation of dust and fumes. Unlike dietary manganese, inhaled manganese bypasses the gut's regulatory mechanisms and enters the bloodstream and brain more directly.

Chronic overexposure to manganese can lead to manganism, a neurological syndrome that causes movement abnormalities resembling Parkinson's disease, including tremor, rigidity, and slowness of movement. Early detection through regular blood monitoring in exposed workers is the most effective way to prevent progression to irreversible neurological damage.

The second common use is monitoring patients on long-term parenteral nutrition (TPN). Standard TPN formulations contain trace minerals including manganese. In patients with impaired biliary excretion, particularly those with liver or cholestatic disease, manganese can accumulate to concerning levels over time.

  • Manganism is a neurological syndrome caused by chronic manganese poisoning. It causes movement abnormalities resembling Parkinson's disease (including tremor, rigidity, slowness of movement) as well as psychiatric symptoms. Unlike Parkinson's disease, it results from external manganese exposure rather than degeneration of dopaminergic neurons.
  • Rarely. Dietary manganese from food and water is tightly regulated by the gut. The main risk of excess comes from inhaling manganese particles (occupational) or receiving it intravenously (parenteral nutrition). Dietary excess in healthy individuals is uncommon.
  • Standard TPN formulations contain trace minerals including manganese. In patients receiving long-term TPN, particularly those with liver or cholestatic disease (which reduces manganese excretion), levels can accumulate to potentially toxic concentrations over time.
  • If detected early and exposure is removed, symptoms may partially improve. However, advanced manganism often results in irreversible neurological damage. Early monitoring and exposure control are the most effective preventive strategies.

How to prepare

Special collection procedures may be required to avoid contamination. Use trace element-free collection tubes. Avoid supplements containing manganese for several days before testing if possible, and inform healthcare provider of all supplements and occupational exposures.

After the test

Elevated levels should be reported to the occupational health team and the relevant workplace health and safety officer. A review of exposure controls, personal protective equipment, and ventilation in the workplace may follow. Neurological assessment is warranted if exposure has been prolonged or if symptoms are present.

Understanding results

Reference ranges for blood manganese vary by laboratory, but levels are generally reported in nanomoles per litre (nmol/L) or micrograms per litre. Levels above the upper reference limit suggest elevated exposure. Significantly elevated levels, particularly with neurological symptoms, raise concern for manganism and require specialist review. Manganese accumulates gradually, so early and regular monitoring in high-exposure workers is key.
