Overview

Magnesium is an electrolyte involved in over 300 enzymatic reactions in the body. It supports muscle contraction, nerve signalling, energy production, bone mineralisation, and heart rhythm regulation. Despite its widespread importance, magnesium deficiency is relatively common and often goes unrecognised because symptoms overlap with many other conditions.

This test measures the amount of magnesium circulating in your blood serum. It is the standard first-line investigation when magnesium deficiency is suspected. Common reasons for testing include persistent muscle cramps, fatigue, poor sleep, heart palpitations, anxiety, and unexplained weakness.

One limitation of serum magnesium testing is that the blood contains less than 1% of the body's total magnesium. The body prioritises keeping serum levels stable, so it is possible for tissue stores to be depleted while the serum result still appears normal. If your serum magnesium comes back within range but your symptoms continue, your practitioner may recommend a red blood cell (RBC) magnesium test, which provides a better measure of intracellular stores.

People at higher risk of magnesium deficiency include those with type 2 diabetes, gastrointestinal conditions affecting absorption (such as coeliac disease or Crohn's disease), long-term use of diuretics or proton pump inhibitors, and those with consistently low dietary intake of magnesium-rich foods.

Suited to people experiencing muscle cramps or spasms, unexplained fatigue, poor sleep, heart palpitations, anxiety, or symptoms that may be connected to magnesium deficiency. Also relevant for people taking diuretics, with diabetes, or with gastrointestinal conditions that affect nutrient absorption.

  • No fasting is required. You can eat and drink normally before your blood draw. If you take magnesium supplements, check with your practitioner whether to pause them before testing.
  • Common symptoms associated with low magnesium include muscle cramps, fatigue, poor sleep, irritability, anxiety, and heart palpitations. Severe deficiency can cause tremors and abnormal heart rhythms. However, these symptoms can have many causes, and a blood test helps determine whether magnesium is a factor.
  • Because over 99% of the body's magnesium is stored in cells and bone, not in the bloodstream. Serum levels are kept stable by the body even as tissue stores decline. If deficiency is still suspected after a normal serum result, a red blood cell magnesium test may give a more accurate picture.
  • People with type 2 diabetes, gastrointestinal conditions (such as Crohn's disease or coeliac disease), alcohol dependence, and those taking diuretics or proton pump inhibitors are at higher risk. Older adults and people with consistently low dietary intake of magnesium-rich foods are also more susceptible.
  • Serum magnesium measures what's circulating in your blood and is the standard first-line test. Red blood cell (RBC) magnesium measures magnesium inside red blood cells, which better reflects the body's intracellular stores. The RBC test is more sensitive for detecting deficiency that serum levels may miss.

How to prepare

No fasting is required for this test. Drink water as normal beforehand.

Let your practitioner know about any magnesium supplements you take, as these may affect results. You may be advised to stop supplementing for a period before testing. A healthcare professional will take a blood sample from a vein in your arm.

After the test

Review your result with your GP or a practitioner familiar with nutritional health. If your magnesium is low, they may investigate contributing causes and discuss whether dietary changes or supplementation are appropriate for your situation.

If serum magnesium is normal but symptoms continue, ask your practitioner about red blood cell magnesium testing, which provides a better measure of tissue-level magnesium status.

Understanding results

Results are reported in millimoles per litre (mmol/L). The reference range for adults is typically 0.70 to 1.10 mmol/L, though this varies slightly between laboratories.

Low serum magnesium (hypomagnesaemia) can be associated with muscle cramps, tremors, fatigue, irregular heart rhythms, and anxiety. Causes include poor dietary intake, alcohol use, gastrointestinal disorders, diabetes, and certain medications including diuretics and proton pump inhibitors.

High serum magnesium (hypermagnesaemia) is less common and is usually seen in people with kidney impairment or those taking large doses of magnesium-containing supplements or antacids.

Serum magnesium reflects only a fraction of total body magnesium. A normal result does not rule out tissue-level deficiency. If your result is normal but symptoms persist, your practitioner may recommend a red blood cell magnesium test for a more accurate assessment of intracellular stores.
