Overview

LDH Isoenzymes testing separates total LDH into five fractions, each predominant in different tissues. This pattern identification helps determine which organ is releasing elevated LDH into the bloodstream.

LDH-1 and LDH-2 predominate in heart muscle and red blood cells. LDH-3 is found in lung and lymph tissue. LDH-4 in kidney and pancreas. LDH-5 in liver and skeletal muscle. The pattern of elevation points toward the affected organ, available across Australia.

This test is useful when total LDH is elevated but the clinical picture doesn't clearly identify the source. It helps focus further investigation on the appropriate organ system.

  • When total LDH is elevated but the source is unclear. Isoenzyme patterns identify whether elevation comes from heart, liver, muscle, blood cells, or other tissues.
  • Normally LDH-2 exceeds LDH-1. When LDH-1 exceeds LDH-2, it suggests heart muscle damage or hemolysis (red blood cell destruction). This pattern was historically used for heart attack diagnosis.
  • Yes. Heart damage elevates LDH-1 and LDH-2. Liver damage elevates LDH-5. Different patterns help identify the affected organ.
  • Some medications cause specific organ damage that affects particular isoenzymes. Inform your healthcare provider of all medications.
  • Isoenzymes rise within hours of tissue damage, peak at 24-48 hours, and return to normal as tissue heals (timing varies by organ).
  • Less common now that more specific markers exist (troponin for heart, specific liver enzymes). Still useful when the source of LDH elevation is unclear.

How to prepare

No special preparation required. Avoid strenuous exercise 24 hours before testing as it can elevate muscle-related isoenzymes. Inform healthcare provider of all medications and recent medical procedures.

After the test

Address underlying organ damage based on isoenzyme pattern results through targeted diagnostic workup, appropriate treatments for affected organs, and monitoring of tissue recovery. Work with specialists based on identified organ involvement.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
