Overview

HIV serology uses a fourth-generation method that checks for both HIV-1/2 antibodies and the p24 antigen in a single blood draw. Antibodies are proteins your immune system produces after exposure to HIV. The p24 antigen is a viral protein that appears in blood earlier than antibodies, which is why this combination approach can identify HIV from around 2 to 4 weeks after exposure.

HIV targets CD4 immune cells and, without treatment, gradually weakens the body's ability to fight infections. Modern antiretroviral therapy can reduce viral load to undetectable levels. Research consistently shows that maintaining an undetectable viral load effectively prevents sexual transmission to partners.

Symptoms

Suited for sexually active adults, people with recent high-risk exposures, healthcare workers after needle-stick incidents, those with multiple partners, or anyone who wants to know their HIV status.

  • Fourth-generation tests are highly accurate (above 99%) and can identify HIV from around 2 to 4 weeks after exposure.
  • When antiretroviral treatment keeps viral load at undetectable levels for at least six months, there is effectively no risk of sexual transmission to partners.
  • Annual testing suits most sexually active adults. People with ongoing risk factors may benefit from testing every 3 to 6 months.
  • A reactive result triggers confirmatory testing. If confirmed positive, modern antiretroviral therapy is highly effective.
  • Pre-exposure prophylaxis (PrEP) is a medication that can reduce HIV acquisition risk. Studies report high effectiveness when taken as directed.
  • The risk from oral sex is lower than from anal or vaginal sex but is not zero. Using barriers further reduces risk.

How to prepare

No fasting or special preparation needed. Let the pathology staff know about any recent high-risk exposures, current medications, and symptoms if relevant. Testing is confidential.

After the test

A reactive result requires confirmatory testing. If confirmed, your healthcare provider can connect you with specialist care. A non-reactive result after a recent high-risk exposure may warrant repeat testing once the window period has passed. Your provider can discuss prevention strategies including PrEP if relevant.

Your test results will be available in your private dashboard. If there are any urgent issues, we'll let you know so you can follow up with your health professional.

Understanding results

Results are reported as reactive (positive) or non-reactive (negative). A reactive result means HIV antibodies or antigens were found and confirmatory testing is required. A non-reactive result means no HIV markers were identified, though infections within the window period may not yet be detectable.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
