Overview

Thyroid antibodies are immune proteins that mistakenly target components of the thyroid gland. Their presence is one of the key indicators of autoimmune thyroid disease, the most common cause of both underactive and overactive thyroid conditions in Australia.

This test measures two specific antibodies: thyroid peroxidase antibodies (TPO) and thyroglobulin antibodies (TgAb). TPO antibodies target an enzyme involved in thyroid hormone production and are strongly associated with Hashimoto's thyroiditis, the leading cause of hypothyroidism. TgAb target a protein the thyroid uses during hormone synthesis and can be elevated in both Hashimoto's and Graves' disease.

An important point to understand is that elevated antibodies do not automatically mean you have thyroid disease. Many people carry antibodies for years without any change to their thyroid function. What antibodies indicate is that the immune system has begun targeting the thyroid, which increases the likelihood of thyroid dysfunction developing over time.

Testing is most useful when thyroid function results are borderline, when symptoms suggest autoimmune involvement despite normal TSH levels, or when there is a family history of autoimmune conditions. It can also clarify the underlying cause when thyroid dysfunction has already been confirmed.

Your GP or endocrinologist will interpret antibody results alongside a thyroid function panel (TSH, Free T3, Free T4) to build a full picture. A positive antibody result on its own warrants monitoring rather than immediate treatment in most cases.

This test checks whether your immune system is producing antibodies that target your thyroid. It's used when autoimmune thyroid disease is suspected — particularly when thyroid function results are borderline, or when symptoms suggest autoimmune activity despite normal TSH levels.

  • Elevated TPO antibodies are a strong indicator, but a diagnosis involves your symptoms, clinical history, and sometimes ultrasound. Your GP or endocrinologist will make the call.
  • Not exactly. TgAb is used in thyroid cancer monitoring because it can interfere with thyroglobulin testing post-treatment. TPO antibodies relate to autoimmune disease. Your practitioner will clarify which is relevant to your situation.

How to prepare

No fasting required. You can eat and drink as normal before your blood draw. No specific medication changes are needed prior to testing, but check with your practitioner if you're unsure.

After the test

Take your results to your GP or endocrinologist. If antibodies are elevated, they'll discuss whether further investigation or ongoing monitoring is appropriate. If you haven't already done a thyroid function test (TSH, Free T3, Free T4), consider adding one.

Understanding results

- Elevated TPO antibodies are the hallmark of Hashimoto's thyroiditis and are also seen in Graves' disease.
- Elevated TgAb can be present in both conditions and is also monitored in people treated for thyroid cancer.
- Some people carry elevated antibodies for years before thyroid function changes. A positive result on its own doesn't confirm disease — your practitioner will interpret it alongside your symptoms and thyroid hormone levels.
