Overview

Thyroglobulin is a protein produced exclusively by thyroid tissue. In a person with an intact thyroid gland, thyroglobulin is present in the blood at measurable levels as part of normal thyroid function. After a total thyroidectomy (complete surgical removal of the thyroid) or radioiodine ablation treatment for thyroid cancer, thyroglobulin levels are expected to fall to very low or undetectable levels, because the tissue that produces it has been removed or destroyed.

This is what makes thyroglobulin useful as a monitoring marker. In people who have been treated for differentiated thyroid cancer (papillary or follicular types), a rising thyroglobulin level after treatment can indicate that thyroid tissue remains or that the cancer has recurred. Conversely, a persistently undetectable level is generally reassuring. Thyroglobulin surveillance is a standard part of post-treatment follow-up and is typically managed by an endocrinologist or oncologist.

There is an important limitation to this test. Thyroglobulin antibodies (TgAb) are present in a proportion of thyroid cancer patients and can interfere with the thyroglobulin measurement, producing falsely low results. When TgAb is present, the thyroglobulin reading may be unreliable, and your specialist will usually order both tests together. A trend in TgAb levels over time can provide additional information in these cases.

This is not a general thyroid health test and is not used for screening. It is specifically suited to people under specialist care following thyroid cancer treatment or thyroidectomy, where ongoing thyroglobulin monitoring is part of the surveillance plan. Results should always be interpreted by the endocrinologist or oncologist managing your care.

How to prepare

No fasting required. Eat and drink normally before your blood draw. If you have thyroglobulin antibodies (TgAb), let your practitioner know before testing — they can interfere with thyroglobulin measurement and affect the reliability of results.
