Overview

The RBC Magnesium test is like having a detective that can peek inside your cells to see what's really happening with your magnesium levels. While standard blood tests only check magnesium floating around in your bloodstream, this test looks directly inside your red blood cells where magnesium actually does its important work – think of it as checking your body's actual magnesium savings account rather than just the pocket change. This gives you a much more accurate picture of whether your cells have enough of this vital mineral to keep everything running smoothly.

Magnesium acts like your body's multi-tool, helping with over 300 different jobs including producing energy, keeping your muscles and nerves working properly, and maintaining steady blood sugar and blood pressure. When your cellular magnesium runs low, you might experience muscle cramps, fatigue, anxiety, sleep troubles, or even irregular heartbeats – but here's the tricky part: regular blood tests might still show "normal" levels because your body prioritizes keeping magnesium in your bloodstream, even if it means robbing your cells.   • Red blood cell magnesium measures the concentration of magnesium inside red blood cells. Since over 99% of the body's magnesium is stored in cells and bone rather than in blood plasma, RBC magnesium gives a much better picture of your actual tissue stores than a serum test.
  • A serum magnesium test measures what is circulating in your blood plasma. The body keeps serum levels stable even when intracellular stores are depleted, so serum results can appear normal while tissue-level deficiency is present. RBC magnesium is more sensitive for detecting this.
  • The RBC magnesium assay requires specific laboratory processing that is available through Healius pathology. When placing your order, you will be directed to find your nearest Healius pathology centre.
  • No fasting is required. You can eat and drink normally. If you take magnesium supplements, check with your practitioner about whether to pause them before the test to get an accurate baseline reading.
  • Consider the RBC magnesium test if you have ongoing symptoms possibly related to magnesium deficiency (muscle cramps, fatigue, poor sleep, palpitations) but your serum magnesium has come back normal. It is also useful for closer monitoring in people with conditions affecting magnesium metabolism, including diabetes and malabsorption disorders.

No fasting is required for this test. Drink water as normal beforehand.

If you currently take magnesium supplements, check with your practitioner whether to pause them before testing to get an accurate baseline.

This test is only available at Healius pathology centres. When booking, confirm your nearest Healius location. A healthcare professional will take a blood sample from a vein in your arm.

After the test

Share your results with your GP, integrative practitioner, or nutritionist. If your RBC magnesium is low, they may investigate contributing factors such as dietary intake, gastrointestinal absorption, medication effects, or chronic health conditions.

If you are actively managing a condition where magnesium status is relevant, such as diabetes or a heart rhythm disorder, your practitioner may use this result to guide supplementation or monitor progress over time.

Understanding results

Results are reported as magnesium concentration inside red blood cells, typically in millimoles per litre (mmol/L). Reference ranges vary between laboratories; your result report will include the applicable range.

RBC magnesium reflects tissue stores more accurately than serum magnesium because it measures what's inside cells rather than what's circulating in the plasma. A low RBC magnesium result, even with a normal serum result, may suggest that cellular magnesium stores are depleted.

Your practitioner will interpret your result alongside your symptoms, diet, medications, and other relevant investigations to determine whether a deficiency is present and what action, if any, is appropriate.
