Overview

ASOT measures antibodies your immune system produces against streptolysin O, a toxin released by Group A Streptococcus during infection. This test helps confirm recent strep exposure when investigating complications that develop weeks after the initial infection.

Rheumatic fever can damage heart valves permanently if not recognised and prevented from recurring. Post-streptococcal glomerulonephritis affects kidney function. Both conditions require documentation of preceding streptococcal infection as part of their diagnosis, available across Australia.

The test is most useful when symptoms appear 1-5 weeks after sore throat and strep infection is suspected as the underlying trigger.

Essential for people with suspected rheumatic fever, post-streptococcal glomerulonephritis, or unexplained inflammatory symptoms following recent sore throat.

Questions

  • Antibodies against streptolysin O, a toxin produced by Group A Streptococcus. Elevated levels indicate recent strep infection.
  • When investigating complications appearing 1-5 weeks after strep throat—rheumatic fever, glomerulonephritis, or unexplained joint inflammation.
  • Generally above 200 IU/mL in adults suggests recent infection, though reference ranges vary. Rising titers on repeat testing are more significant than single values.
  • ASOT typically rises 1-3 weeks after infection, peaks at 3-5 weeks, and gradually declines over 6-12 months.
  • Using both tests together detects over 95% of recent strep infections. ASOT may miss skin infections that Anti-DNase B catches.
  • Diagnostic criteria for rheumatic fever requiring evidence of recent strep infection plus major and minor clinical features.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
