Overview

The tTG-IgA test detects antibodies that form when the immune system reacts to tissue transglutaminase, an enzyme that becomes a target during gluten-triggered intestinal damage. This is the primary screening test for coeliac disease.

Coeliac disease causes the immune system to attack the small intestine's absorptive lining when exposed to gluten. Symptoms range from obvious digestive complaints to subtle presentations like unexplained anaemia, fatigue, or bone density loss. Many cases remain undiagnosed for years, available across Australia.

This test is most accurate when you continue eating gluten normally. Most commonly ordered alongside DGP-IgG and total IgA for comprehensive coeliac serology.

Symptoms

Suited to people with chronic digestive symptoms such as bloating, diarrhoea, or abdominal pain, unexplained anaemia or nutrient deficiencies, a family history of coeliac disease, or other conditions where gluten sensitivity is suspected. Also used to monitor gut healing in people already diagnosed.

Questions

  • Yes. You need to be consuming gluten regularly for at least six weeks before the test. If you have already reduced or eliminated gluten, your antibody levels may be falsely low and the result unreliable.
  • It suggests your immune system is producing antibodies against tissue transglutaminase in response to gluten. This is strongly associated with coeliac disease, but a positive result should be followed up with a gastroenterologist for confirmatory testing.
  • No. A positive blood test supports the suspicion of coeliac disease but is not sufficient on its own. Diagnosis is usually confirmed with an intestinal biopsy performed via endoscopy.
  • IgA deficiency is relatively common and will cause a falsely low tTG IgA result. If IgA deficiency is suspected or known, your practitioner may order a DGP IgG test instead, which is not affected by IgA levels.
  • Yes. Once diagnosed with coeliac disease and following a gluten-free diet, tTG IgA levels should gradually fall. Rising levels may indicate inadvertent gluten exposure.
  • No. Coeliac disease is an autoimmune condition confirmed by specific antibodies and intestinal damage. Non-coeliac gluten sensitivity is a separate condition that does not produce the same antibodies or cause the same type of gut damage.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
