The RBC Zinc test acts as your body's zinc detective, checking these cellular storage units to see if you truly have enough of this essential mineral. Unlike regular blood tests that only measure zinc in your serum (the liquid part of blood), this test looks inside your red blood cells where zinc does its real work supporting your immune system, healing wounds, and keeping your cells functioning properly.

This deeper look matters because your body is quite the master of disguise – it can keep zinc levels looking normal in your bloodstream even when your cells are running low, kind of like maintaining a good-looking storefront while the back warehouse is nearly empty.   • The serum zinc test is the standard first-line measurement of circulating zinc. RBC zinc goes further by measuring zinc stored inside cells. The two tests complement each other, and some practitioners order both to get a complete picture.
  • A morning fasting sample is recommended for the most accurate result, as zinc levels fluctuate with food intake. Avoid taking zinc supplements for at least 24 hours beforehand unless your practitioner advises otherwise.
  • Consider this test if you have ongoing symptoms possibly related to zinc deficiency — such as frequent infections, poor wound healing, skin conditions, or altered taste and smell — but your serum zinc has come back normal. It is also useful for people with gastrointestinal absorption conditions where standard markers may be misleading.
  • Yes. Elevated RBC zinc may be seen with high-dose supplementation. Persistently high zinc can affect copper absorption and cause other imbalances, so this test can identify both ends of the spectrum.

If you take zinc supplements, check with your practitioner whether to pause them before testing to get an accurate baseline. A healthcare professional will take a blood sample from a vein in your arm.

After the test

Review your result with your GP, nutritionist, or integrative practitioner. If RBC zinc is low, they may explore contributing factors such as dietary patterns, absorption issues, or increased physiological demand, and discuss whether supplementation or dietary change is appropriate.

Understanding results

Results are reported as the zinc concentration inside red blood cells, typically in micromoles per litre (umol/L). Reference ranges vary between laboratories; your result report will include the applicable range.

RBC zinc reflects intracellular zinc more accurately than serum zinc because red blood cells accumulate zinc from tissues over their lifespan of around 120 days. A low RBC zinc result, even alongside a normal serum result, may indicate that cellular zinc stores are insufficient.
