Overview

Lipase is a digestive enzyme produced primarily by the pancreas. Its job is to break down dietary fats (triglycerides) into fatty acids and glycerol so they can be absorbed in the small intestine. A small amount of lipase circulates in the bloodstream at all times, but when the pancreas is inflamed or damaged, lipase is released in much larger quantities and blood levels rise significantly.

The lipase blood test is the preferred laboratory marker for investigating acute pancreatitis. It is more specific to the pancreas than amylase (another pancreatic enzyme) and remains elevated for a longer period, typically up to 14 days compared with 3 to 5 days for amylase. Most current clinical guidelines favour lipase as the single best enzyme test for pancreatic assessment. A result more than three times the upper limit of normal in a patient with consistent symptoms is strongly associated with acute pancreatitis.

Lipase can also be elevated in conditions other than pancreatitis. Kidney disease reduces lipase clearance and can raise levels. Bowel obstruction, gallbladder disease, and certain medications including corticosteroids and opioids can also cause elevation. The degree of elevation and the clinical presentation help your practitioner distinguish between these causes.

In chronic pancreatitis, lipase may be only mildly elevated or even within the normal range, particularly in advanced disease where much of the functioning pancreatic tissue has been lost. Additional investigations including imaging and pancreatic function tests are usually needed for chronic disease assessment.

To investigate suspected acute or chronic pancreatitis, assess pancreatic exocrine function, and evaluate abdominal pain or digestive symptoms where a pancreatic cause is considered.

  • Lipase is generally considered more specific for the pancreas and stays elevated for longer (up to 14 days versus 3–5 days for amylase). Most current guidelines favour lipase as the preferred pancreatic enzyme test, though some practitioners order both.
  • No specific fasting is required, but avoiding a very high-fat meal immediately before the test is sensible.
  • Yes. Elevated lipase can occur in kidney disease, bowel obstruction or perforation, gallbladder disease, and with some medications including corticosteroids and opioids. The degree of elevation and your symptoms help distinguish the cause.
  • The two most common causes in Australia are gallstones and excessive alcohol consumption. Other causes include certain medications, high triglycerides, and in some cases no clear cause is found. Your practitioner will investigate the underlying cause alongside confirming the diagnosis.
  • Lipase may be mildly elevated or even normal in chronic pancreatitis, especially in advanced disease where the gland has lost much of its function. Additional investigations including imaging and pancreatic function tests are usually needed to assess chronic disease.

How to prepare

No specific preparation is required for a lipase blood test. Avoid extremely high-fat meals immediately before the test. If you are experiencing acute, severe abdominal pain, seek urgent medical care rather than waiting for a routine blood collection.

After the test

Share your result with your GP. A significantly elevated lipase in the context of abdominal symptoms usually warrants further assessment including imaging. Seek urgent medical care if you are experiencing severe abdominal pain. Do not wait for results.

Understanding results

Lipase is measured in units per litre (U/L). Reference ranges vary between laboratories, but a result more than three times the upper limit of normal is strongly associated with acute pancreatitis in symptomatic patients.

- Elevated lipase: Most commonly indicates pancreatic inflammation. Also elevated in pancreatic cancer, bowel conditions, and kidney disease.
- Normal lipase: Does not completely rule out pancreatitis, particularly in chronic disease where enzyme production may be reduced over time.
- Very high lipase: Levels five times or more above normal in a symptomatic patient are highly specific for acute pancreatitis.

Results should always be interpreted alongside symptoms, clinical history, and imaging when pancreatitis is suspected.
