Overview

The Antinuclear Antibody (ANA) test acts like a security scanner for your immune system, checking whether your body's defense forces have gone rogue and started attacking your own cells. Normally, your immune system protects you from outside threats, but sometimes it gets confused and begins targeting the command centers (nuclei) of your own cells. This test detects those "friendly fire" antibodies that shouldn't be there, serving as an early warning system for autoimmune conditions like lupus, Sjögren's syndrome, and scleroderma.

What makes this test particularly valuable is that it doesn't just give you a simple yes or no answer – it also reveals specific patterns that can point toward different autoimmune conditions, much like how different fingerprint patterns can identify different people. If you've been experiencing unexplained joint pain, persistent fatigue, mysterious rashes, or other symptoms that seem to come and go without clear cause, the ANA test can help determine whether your immune system might be the culprit. While a positive result doesn't automatically mean you have a specific disease, it's a crucial first step that tells you and your doctor whether you need further investigation to catch and manage autoimmune conditions before they cause serious damage.

Symptoms

To screen for systemic autoimmune diseases, particularly when symptoms such as joint pain, fatigue, rash, dry eyes, or dry mouth raise the possibility of a connective tissue disorder.

Questions

  • Not necessarily. A positive ANA is common — around 15 to 20 per cent of healthy individuals test positive, particularly at lower titres. The result must be interpreted alongside your symptoms and other test findings.
  • The titre reflects how diluted the blood sample needs to be before the antibodies are no longer detectable. A higher titre (e.g., 1:320) is generally more significant than a lower one (e.g., 1:40).
  • When ANA is positive, the laboratory reports a staining pattern seen under a fluorescence microscope. Common patterns include homogeneous, speckled, and nucleolar. Different patterns suggest different underlying conditions and guide further testing.
  • ANA is strongly associated with lupus (SLE), Sjogren's syndrome, scleroderma, and mixed connective tissue disease. However, it can also be positive in other conditions and in healthy individuals.
  • No. The ANA test does not require fasting.
  • Your doctor may order more specific antibody tests such as anti-dsDNA (lupus), anti-Ro and anti-La (Sjogren's), anti-Scl-70 (scleroderma), or anti-centromere antibodies, depending on the ANA pattern and your symptoms.

How to prepare

No special preparation is required. Fasting is not necessary. Some medications can affect ANA results, so inform your doctor of any current medications, particularly certain blood pressure, anticonvulsant, or antibiotic drugs.

After the test

A positive ANA should be reviewed by your GP who may refer you to a rheumatologist. Further specific antibody tests (such as anti-dsDNA for lupus, anti-Ro/La for Sjogren's, or anti-Scl-70 for scleroderma) are typically ordered to refine the picture.

Understanding results

A positive result is reported as a titre (e.g., 1:80, 1:160) and a pattern (e.g., homogeneous, speckled, nucleolar). Higher titres are more significant. The pattern can help narrow down which specific autoimmune condition may be present and which follow-up tests to order. A negative result makes a major systemic autoimmune disease less likely, though not impossible.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
