If you've been reading this series, you know it's mostly about blood tests. Standard pathology. Venous blood drawn at a collection centre, processed in an accredited lab, results delivered in a format that GPs and specialists can interpret within a well-established clinical framework.

This article is about something different.

The DUTCH test (Dried Urine Test for Comprehensive Hormones) is not a blood test. It's a dried urine collection performed at home over a 24-hour period. It measures not just your hormone levels, but how your body metabolises those hormones, including the biochemical pathways through which oestrogen, progesterone, testosterone, and cortisol are broken down and cleared.

The honest answer is that the DUTCH test occupies a specific and legitimate clinical niche. For certain questions (oestrogen metabolism pathway assessment, cortisol rhythm mapping, progesterone metabolite quantification, androgen pathway characterisation) it provides information that standard blood tests simply can't. It's not better than blood testing. It's different. It answers different questions.

It's also more expensive, harder to interpret without practitioner guidance, and not necessary for most people doing routine health monitoring. The majority of clinical questions that matter most (thyroid function, iron status, blood sugar regulation, cholesterol, kidney function, liver function) are answered completely and cost-effectively by standard blood work.

This article explains what the DUTCH test measures, when it genuinely adds value, when standard blood testing is sufficient, and how to decide whether it's worth the investment for your specific situation.

A note before we get into it

What the DUTCH test actually is

DUTCH stands for Dried Urine Test for Comprehensive Hormones. It was developed by Precision Analytical in the United States and has become widely used in functional, integrative, and naturopathic medicine internationally.

The method: You collect four to five dried urine samples on filter paper strips at specific times over a 24-hour period, typically at waking, two hours after waking, afternoon, evening, and bedtime. The samples are dried, mailed to a specialised laboratory, and analysed using liquid chromatography-mass spectrometry (LC-MS/MS), a highly accurate analytical method.

How it differs from blood testing: Blood tests measure circulating hormone levels at one point in time. The DUTCH test measures hormone metabolites, the breakdown products produced as your body processes and clears hormones through specific enzymatic pathways. Because hormones are metabolised and excreted in urine, dried urine captures the cumulative metabolic output over the collection period rather than a single-moment snapshot.

How it differs from 24-hour urine collection: Traditional 24-hour urine requires collecting all urine over 24 hours in a jug, which is inconvenient and compliance-dependent. The DUTCH method uses small dried samples at specific time points, which is more practical and also captures the diurnal rhythm of cortisol that a pooled 24-hour collection can't.

What it measures, and why metabolites matter

The DUTCH test measures over 35 hormone markers. Here's what each category reveals and why it goes beyond what a blood test can show.

Oestrogen metabolism

What's being measured

Oestrogen is metabolised through three primary pathways: the 2-OH, 4-OH, and 16-OH pathways. Each produces different metabolites with different biological properties.

Why this matters

The balance between these pathways has clinical implications. The 2-OH pathway is generally considered the most favourable, producing metabolites with weaker oestrogenic activity. The 4-OH pathway produces metabolites that can generate reactive oxygen species and DNA damage if not adequately methylated (neutralised). The 16-OH pathway produces metabolites with stronger oestrogenic activity.

The clinical question

How is your body processing oestrogen? Is the balance between pathways favouring lower-risk or higher-risk metabolites? Is methylation (the detoxification step for 4-OH metabolites) adequate?

Who benefits most

Women on HRT (to assess how exogenous oestrogen is being metabolised), women with a family history of oestrogen-receptor-positive breast cancer (pathway balance may be clinically relevant), women with symptoms of oestrogen dominance, and anyone with elevated oestrogen alongside heavy periods or endometrial concerns.

A blood test tells you your total oestradiol level. That's it. It doesn't reveal which pathway is dominant or whether methylation is adequate. This is where DUTCH adds genuinely unique information.

Progesterone metabolites

What's being measured

Pregnanediol and other progesterone metabolites that reflect total progesterone production and metabolism over the collection period.

Why this matters

Blood progesterone, as covered in the progesterone article, is highly time-dependent (must be tested on Day 21 or 7 days post-ovulation) and fluctuates considerably within a single day. Urinary metabolites reflect cumulative production over the collection period, providing a more integrated picture.

Who benefits most

Women investigating luteal phase deficiency, perimenopausal women assessing progesterone decline, and women on progesterone supplementation wanting to assess metabolic handling.

Androgen metabolites

What's being measured

Testosterone, DHT pathway metabolites (5α-androstanediol, 5α-DHT, androsterone, etiocholanolone), DHEA and DHEA-S, and the enzymes that direct testosterone down the 5α-reductase vs 5β-reductase pathways.

Why this matters

Two people with identical blood testosterone levels can have very different androgen activity at the tissue level. The 5α-reductase pathway produces DHT (the more potent androgen) and the 5β-reductase pathway produces less potent metabolites. Acne, hair loss, and hirsutism are driven by DHT pathway activity, not just total testosterone.

The clinical question

Is your androgen metabolism favouring the potent (5α) or less potent (5β) pathway? Is DHT production elevated even when total testosterone looks normal?

Who benefits most

Cortisol: the full daily picture

What's being measured

Free cortisol at multiple time points (mapping the diurnal rhythm), total metabolised cortisol (reflecting 24-hour cortisol production), cortisone (the inactive form), and the cortisol-to-cortisone ratio.

Why this is superior to a blood cortisol test

As covered in the cortisol article, a single morning blood cortisol is a photograph of a process that only makes sense as a time-lapse. The DUTCH test provides that time-lapse: multiple cortisol points showing the awakening response, daytime trajectory, and evening decline. It also captures total metabolised cortisol, which reflects actual 24-hour production regardless of time-of-day fluctuation.

The clinical question

Is your cortisol rhythm intact? Is your total cortisol production high, normal, or low? Is the awakening response blunted? Is evening cortisol elevated (contributing to insomnia)? Is the cortisol-to-cortisone ratio suggesting altered cortisol clearance?

Who benefits most

People with the "tired but wired" pattern (see sleep article), people with suspected HPA axis dysfunction, people recovering from burnout or chronic stress, and people whose single morning blood cortisol was normal but whose symptoms suggest rhythm disruption.

Melatonin

The DUTCH test measures 6-hydroxymelatonin sulfate, the primary urinary metabolite of melatonin. Melatonin regulates the sleep-wake cycle, and low production can contribute to insomnia, poor sleep quality, and circadian disruption. The DUTCH captures melatonin output from the overnight sample.

This is most useful for people with persistent sleep onset insomnia despite adequate sleep hygiene and normal thyroid, iron, and magnesium levels.

Organic acids and nutritional markers

The DUTCH test also measures several organic acids that serve as indirect markers of nutritional status, including markers related to B12, B6, glutathione, and neurotransmitter metabolism (dopamine and norepinephrine metabolites).

For example, elevated methylmalonic acid suggests functional B12 deficiency even if serum B12 looks normal. Pyroglutamic acid reflects glutathione status (antioxidant capacity). Vanilmandelate and homovanillate reflect catecholamine metabolism, relevant to stress response and adrenal function.

These markers add context to the picture but are supplementary rather than primary diagnostic tools for most clinical decisions.

When DUTCH adds value beyond standard blood tests

The DUTCH test isn't a replacement for blood testing. It's a complement, and it adds value in specific clinical scenarios.

Oestrogen metabolism assessment. If you're on HRT or have oestrogen-related concerns (breast cancer family history, heavy periods, endometrial issues), the pathway analysis provides information blood tests can't.

Cortisol rhythm mapping. If a single morning blood cortisol was normal but you have symptoms suggestive of cortisol rhythm disruption (insomnia, morning fatigue, afternoon crashes, evening wakefulness), the DUTCH cortisol panel is genuinely more informative than a blood test.

Androgen metabolism in PCOS. If blood testosterone is borderline but androgenic symptoms (acne, hirsutism, hair loss) are prominent, the 5α-reductase pathway analysis can explain the disconnect.

Progesterone assessment in irregular cycles. If timing a Day 21 blood progesterone is difficult due to irregular cycles, the DUTCH progesterone metabolites provide a cumulative picture that's less timing-dependent (though collection still needs to occur in the luteal phase).

Complex hormonal presentations. If standard blood work has been optimised and symptoms persist, and the clinical question is about hormone metabolism rather than hormone levels, the DUTCH adds a layer of metabolic detail that blood work can't provide.

HRT monitoring. For women on transdermal oestrogen, progesterone, or testosterone, the DUTCH can help assess how these hormones are being metabolised, not just whether they're being absorbed (which blood tests show).

When standard blood tests are enough

For most people, most of the time, standard blood testing answers the clinical questions that matter.

Routine health screening. The Bloody Good Test or similar panels cover thyroid, metabolic, cardiovascular, liver, kidney, nutritional, and hormonal markers. These are the tests that catch the conditions most likely to affect your health.

Initial investigation of symptoms. Fatigue, weight changes, mood issues, hair loss, skin problems, sleep disruption. The first-line investigation is standard blood work (iron, thyroid, B12, vitamin D, blood sugar, hormones). DUTCH is second-line, after standard markers have been assessed.

Annual monitoring. Tracking HbA1c, lipids, liver function, kidney function, ferritin, and vitamin D over time is the backbone of proactive health management. The DUTCH doesn't replace any of these.

Men's health. Total and free testosterone, SHBG, FSH, LH, and PSA from blood tests cover the vast majority of clinical questions in male hormonal health. DUTCH adds androgen metabolite detail, but it's rarely the first-line investigation.

If your clinical question is "what are my hormone levels?" then blood tests answer it. If your clinical question is "how is my body metabolising my hormones?" then DUTCH answers it. Most people need the first question answered before the second becomes relevant.

How the DUTCH test works

Your test kit is mailed to you with detailed instructions. No collection centre visit required.

You saturate small filter paper strips with urine at four to five specific time points over approximately 24 hours: waking, two hours after waking, afternoon (around 4-5pm), evening (around 10-11pm), and bedtime.

Samples air-dry on the filter paper strips. Once dry, they're stable for mailing with no refrigeration or special handling needed.

Dried samples are placed in the provided return envelope and mailed to the laboratory using the pre-paid label.

The laboratory uses LC-MS/MS (liquid chromatography-tandem mass spectrometry), considered the gold standard for hormone metabolite measurement.

A report is generated, typically within 10-14 business days, with graphical representations of hormone levels and metabolite ratios alongside reference ranges.

For women: Collection should occur in the luteal phase (approximately Days 19-22 of a 28-day cycle) for the most informative progesterone and oestrogen data. If cycles are irregular, discuss timing with your practitioner.

How to interpret results, and why you need a practitioner

The DUTCH report is detailed. It contains over 35 markers, multiple metabolic ratios, and pathway visualisations. It is not designed for self-interpretation.

The clinical significance of a given metabolite ratio depends on your symptoms, your medical history, your medication use, your other test results, and your treatment goals. A 4-OH oestrogen metabolite that looks mildly elevated on the report may be clinically irrelevant in one person and clinically significant in another, depending on methylation status, family history, and oestrogen exposure.

Without a practitioner who understands the test and can integrate it with your full clinical picture, you're looking at data without context. And data without context is noise, not information.

Who can interpret the DUTCH test: Functional medicine practitioners, integrative doctors, naturopaths with advanced training in hormonal assessment, and some endocrinologists and GPs with functional medicine interest. The key is finding a practitioner who uses the test regularly and understands both its strengths and its limitations.

The cost-benefit question

The DUTCH test is more expensive than standard blood work.

When the investment may be justified:

You've optimised standard blood work and still have persistent hormonal symptoms

You have a specific clinical question about hormone metabolism (not just levels)

You're on HRT and want to assess how hormones are being metabolised

You have a family history of oestrogen-receptor-positive breast cancer and want to assess oestrogen metabolism pathways

You're working with a practitioner who specifically recommends it and will interpret the results

When standard blood testing is the better investment:

You haven't done blood work yet (start there, it's more broadly informative and less expensive)

Your clinical question is about hormone levels rather than metabolism

You don't have a practitioner who can interpret the results

You're doing routine annual monitoring (standard blood panels are sufficient and more cost-effective)

For roughly 80% of people, standard blood testing answers the questions that matter. For the remaining 20%, typically those with complex hormonal presentations, treatment-resistant symptoms, or specific metabolic questions, the DUTCH test provides genuinely unique and clinically useful information. Know which group you're in before you invest.

What to do after testing

Common practitioner responses to DUTCH findings:

Unfavourable oestrogen metabolism (elevated 4-OH pathway, poor methylation): Dietary and lifestyle interventions supporting methylation (cruciferous vegetables, adequate B12/folate/B6, reduction of alcohol), and in some cases, DIM (diindolylmethane) supplementation or calcium-D-glucarate.

Low progesterone metabolites: Investigate ovulatory function. Consider progesterone supplementation if clinically indicated (see progesterone article).

Elevated 5α-reductase pathway (androgen): Address insulin resistance if present (insulin drives 5α-reductase activity). Consider anti-androgen strategies if PCOS-related.

Disrupted cortisol rhythm: Stress management, sleep hygiene, circadian rhythm optimisation, adaptogenic support (under practitioner guidance), and investigation of underlying drivers.

Low melatonin: Sleep hygiene, light exposure management, and in some cases, low-dose melatonin supplementation.

Retest timeline: If interventions are made based on DUTCH results, retesting in 3-6 months assesses whether the metabolic picture has shifted. This makes the test most valuable as a before-and-after tool, not a one-off snapshot.
