Overview

TBG (thyroxine-binding globulin) is the main transport protein for thyroid hormones T4 and T3 in your bloodstream. It carries about 75% of circulating thyroid hormones.

When TBG levels are abnormal, total thyroid hormone measurements can be misleading. High TBG means more hormone is bound up in transport, so total T4 may appear elevated even though the amount of active (free) hormone is normal. Low TBG has the opposite effect.

Pregnancy, liver disease, kidney problems, oestrogen therapy, and certain genetic variations can all affect TBG levels.

Useful when standard thyroid function tests do not match symptoms. Relevant for people with liver or kidney disease, those taking medications that affect protein levels, and for interpreting thyroid tests during pregnancy.

How to prepare

No fasting required. Morning collection is recommended. Avoid biotin supplements for 3 days before testing. Let your healthcare provider know about pregnancy, liver or kidney conditions, and medications.

After the test

Discuss results with your healthcare provider to understand how your TBG levels affect interpretation of other thyroid tests. If TBG is abnormal, your provider may rely on free hormone levels rather than total levels.

Understanding results

Normal range is typically 12 to 28 mg/L. High TBG may be caused by pregnancy, oestrogen therapy, liver disease, or genetic factors. Low TBG may be linked to liver disease, kidney disease, malnutrition, or certain medications.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
