Overview

Aldosterone regulates your body's salt and water balance. Produced by the adrenal glands, it instructs your kidneys to retain sodium and release potassium, directly affecting blood volume and blood pressure.

This blood test measures aldosterone levels to investigate resistant hypertension, unexplained electrolyte imbalances, and suspected adrenal disorders. Primary aldosteronism—where the adrenals produce excess aldosterone independent of normal signals—causes an estimated 5-10% of hypertension cases. Many people with this condition take multiple blood pressure medications without adequate control because the underlying hormonal cause goes unidentified.

The test is particularly valuable for Australians whose blood pressure responds poorly to standard treatment, who have low potassium without clear explanation, or who experience fatigue and muscle weakness alongside elevated blood pressure.

Symptoms

Essential for people with resistant hypertension, unexplained low potassium, electrolyte imbalances, suspected adrenal disorders, or blood pressure that doesn't respond to multiple medications.

  • Many blood pressure medications, diuretics, and other drugs significantly affect aldosterone levels. Discuss with your provider which medications to hold before testing.
  • Abnormal results require further evaluation, often including the aldosterone-to-renin ratio and imaging studies. Treatment depends on the underlying cause.
  • Treatment may include specific medications like spironolactone or eplerenone, or surgical removal if caused by an adrenal tumour.

How to prepare

Special positioning and timing requirements may apply. Maintain normal salt intake unless instructed otherwise. Discontinue certain medications as directed. Inform healthcare provider of all medications affecting blood pressure or electrolytes.

If your test needs fasting, we’ll include that in your instructions after you order. Fasting usually means no food for 8–12 hours, but water is fine.
